When most people think of testosterone, they think of men. But testosterone is a critical hormone for women too — and low levels can produce a constellation of symptoms that are frequently dismissed, misdiagnosed, or attributed to stress, aging, or depression.

Why Women Need Testosterone

Women produce testosterone in the ovaries and adrenal glands. While levels are much lower than in men, testosterone plays essential roles in female physiology:

Testosterone is the primary driver of sexual desire in women

Energy and motivation

Supports mitochondrial function and dopamine signaling

Muscle mass and strength

The Symptoms Most Providers Miss

Low libido is the symptom most commonly associated with low testosterone in women — and it is the one providers are most likely to ask about. But it is far from the only sign. Many women with low testosterone present primarily with symptoms that get attributed to other causes:

Unexplained Fatigue That Does Not Improve With Rest

This is one of the most common — and most overlooked — presentations. Women with low testosterone often describe a bone-deep fatigue that is not relieved by sleep, is not explained by thyroid dysfunction or anemia, and does not respond to lifestyle changes. If your fatigue workup has come back normal and you still feel exhausted, testosterone deserves a look.

Difficulty Building or Maintaining Muscle

If you are exercising consistently and eating enough protein but struggling to build or maintain muscle tone, low testosterone may be the limiting factor. Testosterone is essential for protein synthesis and muscle fiber maintenance. Without adequate levels, even a well-designed training program will underperform.

Brain Fog and Cognitive Decline

Difficulty concentrating, word-finding problems, and a general sense of mental cloudiness are common complaints in women with low testosterone. These symptoms are frequently attributed to stress, perimenopause, or depression — and while those can certainly contribute, testosterone deficiency is often an unaddressed piece of the puzzle.

Mood Changes: Irritability, Anxiety, and Low Motivation

Testosterone supports dopamine signaling — the neurotransmitter most associated with motivation, reward, and drive. Low testosterone can manifest as a persistent low-grade depression, loss of motivation, increased irritability, or a general sense of "flatness" that does not respond well to antidepressants alone.

Changes in Body Composition

Increased abdominal fat, loss of muscle definition, and difficulty losing weight despite diet and exercise are all consistent with low testosterone. Testosterone influences insulin sensitivity and fat distribution — without adequate levels, the body tends to store more fat and burn less.

Many standard lab panels do not include free testosterone — only total testosterone, which can be normal even when free (active) levels are low

Reference ranges for female testosterone are wide and often outdated — a result that is "in range" may still be suboptimal for that individual

Symptoms are frequently attributed to stress, depression, or perimenopause without investigating hormonal causes

Testosterone therapy for women is not FDA-approved (though it is widely used off-label), making some providers reluctant to prescribe it
